SPORTS PHYSICAL EXAMINATION IINFORMATION for 2008-09 SCHOOL YEAR

   The School Physician will offer sports physical examinations on May 23rd to 5th, 6th and 7th grade students. There will be team sports for grades 7 and 8, and intramural sports for grades 6, 7, and 8. Students should report to the gym immediately after school.  Eighth Grade students participating in a FALL sport will have their physical examinations at Cranford High School on June 6th.  Eighth Graders should report to the upper gym at Cranford High after school on June 6th.  The following procedures should be followed:

Beginning April 9, 5th, 6th, and 7th grade students who plan to participate in any fall, winter, spring or after school sport during the 2008-09 school year should report to the school nurse for a sports physical form (Part A and Part B). Only 8th grade students who will participate in a FALL 2008 sport should report to the nurse for the forms. Part A should be completed by the parent/guardian and returned to the school nurse.  The nurse will then complete the height, weight, blood pressure and visual acuity on Part B of the form.  The 8th grade forms will be sent to the high school.  The deadline for the completion and return of Part A to the school nurse is Thursday, May 1 for 5th, 6th and 7th graders, May 28th for 8th graders.  An Athletic Participation Consent Form must be completed by the parent before the physical examination and for each sports season.

   A Health History Update will be required if a sport begins 60 days after the initial physical. The school nurse or the coach will distribute these updates each season.

   A Steroid Testing Consent Form must be completed by all 8th grade parents before the physical examinations are done at the high school.

   Either the school physician or a student’s private physician may conduct sports physical examinations.  The appropriate NJ forms must be completed. If your physician does the physical, the school doctor must cosign the physical before approval is given.

   Physical exams will be good for 365 days.
