Documents and Fees Required for Licensure:

• Application for Licensure: Complete Part A, #1-17 and Part B, #1-16 Oath of Allegiance must be notarized. Out-of-state residents may have the Oath of Allegiance notarized in their respective states. In Part A, #5A, list the license title(s) for which you are applying (see list of available endorsements). Complete Part A, #5B if you are applying for evaluations only. Complete Part A, #5C and or #5D if you applying for duplicate license(s) and/or name change(s).
